We collect information directly from you: in person, over the phone, on intake and medical history forms, and through the contact form on this website. With your consent, we may also receive records from a previous dentist or another health provider involved in your care.

Messages submitted through our contact form are transmitted and stored by our website host to deliver them to our team. Please don't include detailed medical information in the form.
